About Closed Loop Medication Management System

The Closed Loop Medication Management (CLMM) System is a revolutionary process that integrates automated and intelligent systems to completely close the inpatient medication management and administration loop. By digitalising this process, this Smart Health system has significantly enhanced patient safety and improved efficiency of the entire medication management process, from prescription ordering and supply to the administration of the medication.

The CLMM system was rolled out by Synapxe to KK Women's and Children's Hospital, National University Hospital, Tan Tock Seng Hospital, Changi General Hospital, Institute of Mental Health, Khoo Teck Puat Hospital, Singapore General Hospital and Teng Fong General Hospital.

Opportunity

Prior to the automation of processes, nurses previously spent substantial time on medical administration. Pharmacists had to manually review prescriptions and check patients’ details to prevent medication errors and adverse drug events.

Solution

CLMM integrates automated and intelligent systems such as the Electronic Medical Records and Clinical Decision Support systems, where doctors enter medication orders and are prompted with information to avoid adverse drug events. Pharmacists verify medication orders, which are then packed by the Inpatient Pharmacy Automatic System. At the wards, nurses scan QR codes or barcodes on the packed medication to match them to the right patient.

Impact

CLMM reduces time spent on medication administration, allowing nurses, pharmacists and doctors to spend more time on patient care. Patient safety is improved as doctors are alerted to recheck their prescriptions and are notified of potential adverse drug events. Medication inventory management has also improved, reducing annual holding costs by 30 per cent. The data from CLMM systems can also be analysed to improve medication choices and patient outcomes.
